The Effectiveness of a Mobile Application Based on Verb Network Strengthening Treatment to Improve Sentence Production in People with Aphasia

Master Abstract

Introduction: Aphasia is a neurological language disorder resulting from damage to the parts of the brain responsible for language. This leads to difficulties understanding and producing language in general, and difficulty understanding and producing verbs and sentences in particular. This impacts the communication of aphasia patients with others. Therefore, verb network strengthening treatment has emerged, a treatment based on activating the semantic network associated with verbs, which contributes to enhancing sentence production and improving functional communication. Digital language rehabilitation based on modern technologies, such as mobile applications, is a recent trend in the rehabilitation of aphasia patients. Mobile phone applications provide mobility and movement for treatment, offer treatment anywhere, anytime, and provide the necessary interaction to improve treatment outcomes. Study Problem: The researcher came up with the at the problem of the study as explained below: The researcher observed numerous instances of verb and sentence production impairments through her work as a teaching assistant in the Department of Language and Speech Disorders at the Faculty of Special Needs Sciences at Beni Suef University. She also conducted field visits to hospitals and language rehabilitation centers in Hurghada, Red Sea Governorate. Through a survey and analysis of speech samples from a sample of aphasia patients, the researcher verified the presence of verb and sentence impairments among aphasia patients. The researcher also reviewed numerous previous studies and literature that confirmed the selective deficits in verbs in people with aphasia (Miceli et al., 1984; Zingeser & Berndt, 1990; Damasio & Tranel, 1993; Berndt et al., 1997a, 1997b; Kim & Thompson, 2000; Thompson & Shapiro, 2007; Wahlstrand & Saldert, 2025; Sazegar Nejad et al., 2025). Therefore, the researcher turned to a therapeutic protocol to improve verb and sentence production in people with aphasia. It was found that there was a scarcity of studies addressing this issue with Arabic-speaking people with aphasia, which sparked the researcher's curiosity to measure its impact on a sample of Arabic-speaking people with aphasia. However, the researcher faced a challenge: the difficulty of transporting sample members to receive treatment for several reasons, including the mobility difficulties that may accompany aphasia, the shyness of aphasia patients to leave their homes and confront society while experiencing communication difficulties, and their advanced age, which may cause them to suffer from numerous health problems. Therefore, the researcher sought to provide a treatment protocol for strengthening the verb network using modern, easily portable digital technologies to provide treatment anytime, anywhere, such as mobile phone applications. Through the above, the study problem can be formulated in the following main question: What is the effectiveness of a mobile application based on verb network strengthening treatment to improve sentence production in people with aphasia? The researcher will seek to answer the study question by answering the following sub-questions: 1. What are the design criteria for a mobile application that provides language rehabilitation services for people with aphasia? 2. What is the educational design of a mobile application based on verb network strengthening treatment to improve sentence production in people with aphasia, given the design criteria according to the ADDIE model? 3- What are the differences in pre- and post-tests in sentence production among individuals with aphasia after training individuals with aphasia using a mobile application based on verb network strengthening treatment? 4- What are the differences in post-tests and follow-up in sentence production among individuals with aphasia after training individuals with aphasia using a mobile application based on verb network strengthening treatment? Study objectives: The current study aims to: 1- Develop a list of design criteria for mobile applications to provide language rehabilitation services for individuals with aphasia. 2- Design a mobile application based on verb network strengthening treatment (VNeST) for Arabic speakers with aphasia, for use in language rehabilitation. 3- Evaluate the feasibility of using verb network strengthening treatment for individuals with aphasia in an Arab environment, and evaluate the feasibility of using mobile technology in designing language rehabilitation treatments for individuals with aphasia. 4- To ensure the continued effectiveness of a mobile application based on verb network strengthening treatment to improve sentence production in people with aphasia after its completion and during the follow-up period. Significance of the Study: The current study has significance at the theoretical and applied levels, and can be explained as follows: Theoretical Significance: 1. Focus on a group of individuals with special linguistic and psychological characteristics: aphasia patients with selective verb deficits, given the scarcity of Arabic research addressing this group. 2. Enriching the Arabic literature: The study provides an important scientific contribution in bridging the clear gap in Arabic literature on verb network strengthening treatment (VNeST) and its impact on people with aphasia, as well as the clear lack of use of technology and digital techniques in language rehabilitation. Practical Significance: 1. Develop a standardized electronic diagnostic test and make it available as a tool to diagnose the comprehension and production of verbs and sentences for people with aphasia. 2. Develop a list of criteria for designing mobile applications for language rehabilitation for people with aphasia, allowing access to and use of mobile technology by individuals with aphasia. 3. Develop digital therapeutic tools: By designing a therapeutic application for the rehabilitation of people with aphasia in Arabic, based on scientific design principles based on design standards and therapeutic principles based on verb network strengthening treatment, this provides a practical tool for speech therapists, caregivers of people with aphasia, and specialized care centers. 4. Provide interactive therapy while facilitating access to therapy for people with aphasia and their families, improving their communication with others and, consequently, their quality of life. Study Sample • Exploratory Study Sample: The first exploratory study sample consisted of (5) individuals with aphasia, and the second exploratory study sample consisted of (22) individuals without aphasia. • Psychometric Properties Sample: The psychometric properties validation sample for the electronic multimodal verb and sentence test for people with aphasia (prepared by the researcher) consisted of (40) individuals without aphasia and (30) individuals with aphasia. • The primary sample of the study: The primary sample of the study consisted of 7 individuals with aphasia. Statistical Methods • Descriptive statistics using means and standard deviations. • The Kolmogorov-Smirnov test and the Shapiro-Wilk test to verify normality. • The Pearson correlation coefficient. • Cronbach's alpha coefficient to verify test reliability. • Spearman-Brown coefficient and Guttman coefficient to verify split-half reliability. • Independent sample t-test. • ROC operating characteristic curve to calculate the test's diagnostic ability and determine the cut-off point. • Z scores and T scores were calculated from the raw scores. • Wilcoxon test. • Effect size was determined using Cohen's t-test. 2. Spatial limitations: The study was administered in the homes of the patients with aphasia, the study sample, after they had been discharged from the hospital and had been stable for at least two months, according to a report from the specialist physician. Care was also taken to ensure that the study was administered in a quiet room. 3. Temporal limitations: The study was administered over a period of one year, from 2024 to 2025. Study Results: The study results showed that: 1. There are statistically significant differences between the mean scores of the study sample members with aphasia in the pre- and post-tests in sentence production using the electronic multimodal verb and sentence test for aphasia patients, in favor of the post-test. 2. There are no statistically significant differences between the mean scores of the study sample members with aphasia in the post-tests and follow-up in sentence production using the electronic multimodal verb and sentence test for aphasia patients. An analysis of the results of the experimental cases also showed a spread of activation of the verb networks from the trained verbs to the untrained verbs semantically related to the trained verbs, and a spread of activation from them to the untrained verbs that are not semantically related to the trained verbs. This sequence of activation with semantic roles was evident in most cases. The analysis of the case results also showed a marked improvement in the production of sentences with one semantic role, two semantic roles, and three semantic roles.
